   Dog owners: After you fill out and submit this form, you will be contacted with instructions on how to pay the annual fee. Once we have received your payment, your pet tag and paper license will be mailed to you.

   Cat owners: By submitting this online form, a paper license will be automatically mailed to you. If you area a new registrant, a pet tag and paper license will be mailed.
