    Please ONLY submit this form when your department Accessibility Champion is unavailable and you are unsure how to remediate the document. Also please submit the NATIVE FILE that the document was created in. For example if it was created in word but converted to a PDF please submit the WORD document. Therefore PDF should not be submitted that often.
