Requesting a Record

The Frederick Police Department is in compliance with the provisions of Colorado Revised Statute 24-72-301 regarding the release of Criminal Justice Records. We may, according to statute, refuse the release of certain records as prescribed in the law. 

You can fill out the records request and submit it online.

You may request a copy of your records at any time, and the Frederick Police Department will be allowed at least 24 hours to process each individual request. 

  • You will be expected to show a valid driver's license or provide valid identification.
  • You will be required to sign a record request form.
  • You will be required to pay the current posted fees.
  • You may fax in writing a records request form to 720.382.5720, but you will still be required to come into the Frederick Police Department at 333 Fifth Street with the proper fees to collect your report.
